- Immunizations or Declination
- Must sign an Enrollment Packet
- Enrollment Packet Includes State Required: Enrollment Agreement, CSC-005, CSC-010, Declination, Tour Form
State Exam Requirements: 1 Exam (RDA)
Minimum Age: Yes—only with a HS diploma, can a person apply at 17 (with parent permission, signed on the Enrollment Agreement). This is part of the requirement to take the RDA Exam with THSCSA.
Radiography: RDA Exam
Credentials on Completion:
- “Certificate of Completion”
- Basic Life Support (BLS) Certification—Certified through the American Heart Association
- RDA “Registered Dental Assistant” (Only after taking the Exam & receiving your RDA from the TSBDE—Texas State Board of Dental Examiners)
